If you want to have a data-entry page, you could use content controls to capture the input, combined with linking and/or ContentControlOnExit macros to replicate the data elsewhere in the document. See: http://gregmaxey.mvps.org/word_tip_p...ting_data.html

If you don't want a cover page for the data entry, you could do the input via a userform and use that to populate the document. The above link has some information on that, too.
