Need help in vba coding please (macro application)

Good Day to everyone. I'm Jerome and I am new here. It is nice to know that there is this kind of forum where everyone can ask regarding excel.

I know little in excel that is why i am seeking your help in this matter.
Could someone help me with coding the attached file. I tried my best effort but it is still not enough. I have already followed some video tutorials but every time I run the data entry to post the data in the worksheet, It still a failure. Could someone help me this or provide me tips and if it is ok provide code for these. I have attached the workbook and I have already created the USERFORM. All i need is to code. Please help me. Your help is very much appreciated.

I didn't find any userform in your workbook. So created a simple one. You change it as per your need.
After Alt+F11, double click on Submit button, you will see the code to enter the data in the sheet. You can input data for all your columns but you need to add the code accordingly. Please see the attached sheet.

Dear Sir Sktneer:

Could you please help with below concern:
• I want to add update button that enables me to update time to time the details of the monitoring. Since, some data are not yet available when entry will be made at first. What shall i do, do i need to put search button or somehow combobox button, if yes, could you help me with the code? thanks and God Bless!
