Capturing dates in Table of contains from discountinous rows

    Hello,

I am trying to develop a pateint scheduler for which I need your help.

I have added few dummy patients in the sheet. The sheet is named after these patients number.

Now my issue is , whenevr I will add new sheet, the sheet name should get added in the first sheet name "TOC" starting from cell B11.
And when I delete any sheet that particular sheet details should get erased from "TOC".Remember I have hidden a Template sheet which should not get added to TOC along with TOC itself in that list.
The list should contain only the patient number sheets name.

Also, in addition to this, if it is possible, the scheduled date should appear next to sheet name in sheet "TOC" from cell C11.

The schedule date is in all the sheets. The idea is first the schedule date will be decided and enetered (which should appear in TOC) then the patient will attend the visit and will enter the actual date.
Once the schedule and actual date is enter the treatment will be completd and then the next treatment date will be scedule (which again should appear in TOC, as the actual date for the previous treatment is entered)
So the schedule date should appear in TOC only when the previous treatment actual date is entered.
The challenge here would be the treatments are divided into 3 phases. Again the 2nd phase into 3 part (In order to print the sheet)

So the treatment are in continous manner an will appear & schedule one after the other.

Note: The code should take the dates even though there is a formula in teh scedule dates cells.
Hope I will get a positive reply.

    Thanks in advance.
