User Forms of Data Entry Area that feeds multiple output Areas- drop-down validation

I have been seeing a lot in this forum in reference to user forms and I would like to know if anyone is using a user form for data entry that can feed multiple output files. I have attached an example of what i am looking for. I can create a user form but it is difficult for me to understand how it works in a project like this. Any help would be greatly appreciated.
