Connect & Categorize Data by a Common Number

Can I create a Macro to do this for me?

I've got two worksheets. The first has a list of numbers associated with a specific person. This list remains constant from month to month with occasional changes. The second is a list of dates associated with a number. When a number appears on the second worksheet(month to month) I want it to pull the name of the individual associated with that number. I would also like to attach the text associated with the date and number when it is matched with a person. I've attached an example of both lists. Thanks in advance for any advice and/or direction.
