HTML or a Web Form using Excel as the database....

Hi All,

I’m requesting for help to Create a Web / HTML Form.

1. I get a CSV / XLSX file daily which has some data (Demographics part of the attachment)
2. Request is to be able to upload this file on an interface. Each entry can be filtered by using the unique code in each line using a dropdown menu
3. After filtering I’ll need to deal with each entry individually my team needs to plug in the feedback (The questions in the attached Feedback form should appear on the web form similar to a survey link / feedback form)
4. Once the data is fed I should be able to extract the complete dump in the form of a CSV / XLSX file with details

I’m not sure if this can be done, and if this is the right place to post this query.

I’ll appreciate if someone could help me create this or direct me to the right path.
