Need help to combine 5 difference cases in CONCATENATE formula

I'm having a difficult to combine all 5 differnce cases into 1 Concatenate formula. Please see the sample in attached and thank you in advance.

I put your code in new WB and got an error (FALSE at the end of each case). Can you take a look and help me out.

Hi There! The formula contains all 5 cases (up to my understanding) All there is to do is copy it for each line entry.

Hello RCM,
There's notthing wrong with your code but when I modified to fit my wb then I got "FALSE" in the middle of case 2 and case 3.

The code works now because I was missing "," and thank you very much for your tiem and help.
