Newbie, need help with Macro to duplicate template sheet

First off, I am extremely new to VBA so I appreciate any help I can get. Here is what I am trying to do.

I have a workbook with one sheet (Template). I would like a macro to create duplicate copies of the (Template) sheet and name each of the new sheets using the following naming format. ("dd-mmm-yy"). I have a macro that will create the new user defined sheets which I like, however, I don't have a clue how make it also copy the contents including the formating to the newley created sheets. Would anyone be willing to help me out? Thanks in advance.

Yes, that is exactly what I had asked for. Is there a way to perform the same thing without copying the the form control button to all the sheets?
