Copy and Paste Values Base On Criteria

Hi,

Does anyone know a macro that I can assign to the form such that when the user keys in the value in cell C10, I10 and F10 and clicks on the copy button, and the value will be copied and paste into the respective table below based on the criteria of month and year in C6 and D6.
