Multiple checkboxes to generate filtered extract

My apologies if my thread title is repetitive or not clear enough. I will try to explain in detail though.

I have a macro that generates a pre-filtered extract depending on the user selection from a couple of drop down lists. The extract has month or quarter wise information among other details.

Here is the requirement: In the attached mockup (row #10), I have included 4 Active X Controls checkboxes (1 for each quarter). I want to give the user the flexibility to select the desired quarter before the pre-filtered extract is generated. This is typically how the sequence should be:

1) When the user hits "Generate List1" button, a message box will prompt the user to select the desired quarter in row # 10. Basis of his selection (could be 1 quarter or multiple quarters), the extract is then filtered for the quarter and output generated.

Would appreciate any guidance or the correct method to do this. I am stuck since there are multiple checkboxes to deal with.....
