Collecting survey data from radio buttons

I was wondering if it was possible to automatically collect data from radio buttons in an excel workbook. I have a survey that was sent out 300+ individuals that has multiple radio buttons and I was hoping there was a way to automatically collect the information.

I've included an example of the survey as well as an example output worksheet of what I was hoping to collect back to compare data. In the example output I was thinking that it would be nice to somehow grab the file name and list it out since people will be changing the file name with demographic info that I could use though I don't know if this is possible. The survey that was sent out doesn't include the example output worksheet.
