Macro with pop up user input and radio buttons

I have a macro that combines some data, and then reformats the data. There are a couple of options of reformatting the data that are required, but this is based on user input. It's more than just a yes/no, I would like a few selections of radio buttons, and then depending upon what the user selects, certain formatting will occur.

Being relatively a newbie, I cannot seem to find the code for this user input box. Any help and specifics would be greatly appreciated!
