Macro to move data from one sheet to another

Hi, first time poster here so I hope I've explained the problem clearly.

I need a macro that moves data from one sheet to another if certain conditions in the first sheet are met - a mock up is attached which hopefully shows what is needed.

This has been driving me mad so any help would be really appreciated.
