Pardon me for my naivety among such matters, I took over a spreadsheet for a hiking club and although I'm leaning somewhat, I can barely manage data entry.
We have a number of columns with different data, including names, address, phone and other contact info along with other info. One of the macros pulls the data and makes registration forms depending on the entry. The form is kind of sloppy and I'd like to clean it up some and add our logo.
I've attached the pdf and jpg of the Word docs to show you what I'd like to do. Hopefully you can help.
