Converting unorganized raw data dumped into 1 column into multiple organized coluns

I received a csv file from a database that contains multiple records and multiple pieces of information per record. There does not seem to be any specific organization to this file - aside from sporadic headers contained within a cell of text... ie: Main/Home: (xxx) xxx-xxx .... which displays the phone number of the person included in that record. I need to convert this information into an organized file for a data migration. I've tried to copy/paste into word and convert to a table, i've tried copy/paste special - transpose in excel, i tried using a pivot table. I'm not even sure it's possible to clean this up without purely re-entering each data point manually. However, this file has 225,000+ rows right now.... so I'm willing to ask for any help, suggestions, advice anyone can give me. I do use arrays on a regular basis in other work I do in excel and feel fairly comfortable with most of the tools in excel.

I am attaching the original csv file saved as an excel workbook cropped down to 500 rows and edited slightly to prevent the posting of personal data (elrobinson data file) - as well as posting the template i'm needing to convert to for data migration (elrobinson convert to).

    Thanks for any help!!

hi elrobinson, welcome to ExcelForum, attempt (works on the sample file provided for required fields only), save both files, unzip .csv file, open .xls file and press Run, select .csv file to process

Files reposted. Zip folder contains csv file. Alternatively you can save txt file and change it to .csv

I saved both files - but when i tried to unzip the data file, i got an error message that the compressed file is empty. Is there something I'm not doing correctly?
