Input Box with Multiple Inputs

Hi All!

I'm looking for assistance making a macro that functions like this:

Where the white boxes are input boxes and will store to a value.

Cancel will exit macro.
Next job will add the input values to a variable and reset the input boxes so that additional numbers could be added.

Finish & Submit would then post the values in a desired cell range and end the macro.

If someone would help with this I would be sooooo appreciative!!!
