    Lookup with multiple criteria on a separate sheet

    I need to be able to look up the patient name, date of service and drug on one sheet and return true if it shows up on another sheet. I have attached the example that I have used that is not working. I have opened a previous thread on this but I was unable to figure out how to attach a file to that thread so I opened this one. Thank you.
