medical calculator help

hi guys, hope you can help me with this.

ive managed to program a simple medical calculator for (LVMI-left ventricular mass index) that i use in my profession as a cardiologist.

however i need excel to somehow categorize the values based on 3 variables (colored red in my worksheet attached here)
1. gender = m/f
2. LVMI
3. relative wall thickness

the categorization is a simple table (picture also attached in worksheet) that would go under the "description"" (colored blue)
they would fall under 4 categories based on the above values as shown in the table
1. eccentric hypertrophy
2. concentric hypertrophy
3. concentric remodelling
4. normal geometry


ive tried using lookup formulas however the table would need more than just two columns to reference the values,

any help would be greatly appreciated. thanks!
