Chart w/ 2 drop downs! Help me out please!

I've attached the spreadsheet that I'm working on and I actually need it for work tomorrow morning. So please help me out if you can!

At the top, I've created a graph with a dropdown of providers. As you can see, it populates the table right below the graph and it pulls data from right below there. No problem there. Well, I was asked to do this but being able to break it out by both Provider and Month. So I need to be able to select Provider & a month! Starting at line 60, below the black line, I started working on it, but I cant figure it out. Below that is all of the data (provider, month, each code in Signature, each code in Allscripts). In the graph, I need to see 3 lines when I select a provider--- Signature, Allscripts, Average. Can you please help me out??? I would really appreciate it.

Have a look at the attached. I've changed the way the drop downs select the data, so hopefully that will give you some hints.
