Copy data one below the other

Hey all,

I receive this input file (attached) every week which is in the particular format. I need to convert this into an output file for further analysis.

The report needs to be created in this format -
- Copy columns A to C. Copy the contents of column D.
- Copy columns A to C (below the above data) and then copy the contents of column E.
Proceed till I.

The output file format is also attached. Whatever value is available in row 1 for columns D to I should appear in column E. Column A and B should have Mexico and LN13 in all rows respectively.
