Advanced filter with user form

I am attempting to program a user form to operate an advanced filter, the object being to only apply the filter for the fields if completed and anything left blank to be ignored, two of the fields are in list boxes as there could be cause to select more than one.
I have made a mess of the code and could really use some help.
