Auto hyperlink

Hi,
I have a series of data in Column C and Column H.
Also I have individual worksheets based on the datas named in Column C and H.

I want to have a button with name Generate. and when clicked it will automatically create a hyperlink based on the datas available in column C and Column H to Column Q and Column T.
That is when I click the link it will take me to that sheet.
I have highted the place in red where the hyperlink to be generated.

I am attaching a dummy sheet for reference.
