Transpose Columns To Rows (with Variable Columns)

Hi

Please find attached a sample of my data and how I need it arranged. My end goal is that I have one cell of notes per member.

I have tried to use pivot tables and IF functions but I don't know enough about excel to do what I need.

Any help would be great.
