    Macro To Transfer Form to New Sheet and Data to Another Sheet

    Hi All,

    I need help in creating a form that will consolidate and collect all of the data I need for my current project.

    On the Form itself, there would be a field for Name, Date, a Free Form Comment and Scores for 12 Questions.

    Every time the button for execute is hit, the form (the whole form not only the data entered) should be transferred to a new sheet titled using the data entered on the Name Field. The data entered for each field should also be transferred to the Raw Data sheet on their corresponding columns. The data on the Form Sheet should also be Blanks and ready for another entry. Any subsequent entries should go to the next empty row on the Raw Data Sheet.

    Any help on this would be greatly appreciated.

    Thank you. This Actually helped me get started. I'm going to look for a way in creating a template from this.
