Dashboard for dynamic list

Hi -

I've trawled the internet for an answer to this problem with no luck, but I'm sure with your immense knowledge you guys will have the answer.

Basically, I have a provider list offering different types of activities and I'd like a dashboard where you can select an activity type from a drop down menu and this will display the provider details that offer that particular activity.

I enclose a sample of the provider list for information.

It sounds simple - probably is - but I'm absolutely stumped!

Any help greatly appreciated.
