Multiple Criteria Lookup in VBA

I want to return the value in column C based on two user-defined fields that will be contained in column A and column B. For example, for company1 and field3 I want to return '54' and for 'company3' and 'field5' I want to return 'Pig'. The Excel data layout is shown in the attachment. I am happy to hard code the two search criteria (Company and Field).

Once that is sorted I would to map the value to pre-defined bookmarks in Word. The bookmarks match the names found in column B.
