Setting up the Excel files for Configuring Website Logs

I made this

The idea of that file is that i want my friends to put all their website login (user and pass) into the Virtual World Folder, so they can manage each website they signup to.

Now, want i want to do is i want to make a Form actually some type of Databse to put Username, Password, Refferal Url, Website Url, E-mail Used, etc.

Can someone help me on this ?
