form with a variable set of controls

I have a list of text items in say A1:A8, however that range can change, so I'll call it rngKeyDateNarrative.

I have a form which i'll call frmCPDates.

On this form, for each item in myRange :,
i need 1 textbox, 2 date fields (poss textboxes as well), an option button, and a checkbox

And I haven't a clue where to start.

I have a file called "CP Tool v0_3.xls".

I'd like the workbook to check (and advise) if a higher version exists (i.e. "CP Tool v0_4.xls" etc.

I could hard code it, but of course the version will frequently change.

Has anyone done this before?
