vba moving userform to database

i created a very basic userform... it has 15 input values from combo boxes.. when i am moving them to the database with the following code.. only the first 13 go.. the last 3 do not.. there is an entry for them?? any help will be greatly appreciated!!! I'm not sure why they don't copy to the database??

Your code should work, can you attach the workbook containing the form?

thanks so much for responding.. it actually was user error.. i had the cells that it should have been copying to hidden... so i didn't see it... :-) but you are right.. it was working the whole time....

my frustration was user error!!!!! hahaha!!!!
