Please help me create pop up questions

I need to create a questionnaire so that a question pops up along with a spot to answer the question. Once the question is answered, the next question pops up, etc....

Obviously, from there I would need the answers to populate a spreadsheet.

Help please!!!

You might check out the VBA Help files on MsgBox and InputBox and see if
they spark any ideas.
