Submit button macro to store data then clear form

Hi Everyone

I've attached a spreadsheet I have created with a submit button.

What I want to do is create this as a working scorecard where when I press submit it transfers all the information into the datastore worksheet and then clears the scorecard so I can mark multiple times. and each time I press submit it adds a new line into the datastore.
