    User Form to Add Data to Table

    Hi Guys,

    I need help on formulas for the uploaded excel sheet work out.
    The idea is to create a form to capture data and after cliquing on validation button, the new entry will automaticaly be pasted to data base according to the selected date.
    See more details in the excel sheet.

    You have posted in the VBA section, but are asking about formulae - which type of solution do you want?
