Data form in excel

Dear Colleageus;
i am not sure weather it is doable in excel or not but asking for your assistance to guide if this can be done in excel.

I want to develop a simple form, which user can use to enter the data and on the backend it will also maintain the database. moreover, while filling the form it will also have a coloum for service referal which will have several checkboxes options and any of the checkbox select would generate an email to some specific email based on the checkbox select.

please let me know if anything is unclear or ambiguity.

Thankyou so much logit for your response and confirmation. as of now i have only develop a simple excel sheet that contain coloum of question to ask in survery. However, i would be grateful if you could guide me from where and how i can start rest i can search over youtube for help or any other forum you may suggest.

in my case this is not possible because i have asked from forum expert to guide me that how i can develop a form and then add. so please if you can guide me on how i can develop the form or any link which can guide me. then i can follow it
