Dynamic User Form

Dear All
i have big sheet with many data entry columns.
i need to help me to make user form for it even with paid amount for it.
please check attached file for details.
it has user form it is simple but i can not integrate it with the sheet..
Sheet includes combo drop down list
multiple selection buttons
with different pages.

Cut the file down to 20 to 100 rows on each page.
Without the file structure and present layouts/forms it is impossible to help.
