vba macros to copy send data from 1 sheet to another with specific information

Hello again,

I am looking for a macro where I have entered a data on worksheets (Form) and return into another sheet (Database) within the same workbook.

I have search for many topics which is having 1 record can be copies to the other sheet but my logic is different.

I have specific information which need to copies with each row data while copying to the other sheet.

For example: I am entering a client items with multiple items. So, each item I need to copy the same client details as showing in the attachment.
I appreciate if any of expert can help me to do this as I have to register daily 5 to 6 thousand client’s information with multiple details.

Of course the attached is the sample data which is not actual data.
