How to create useform in excel VBA to search and display data on same form?

Features:
Search the data using ID and D.O.B
and it automatically displays other related information.

The attached file is one approach that may help if you are considering using a UserForm/VBA.
It can be readily adapted as I have not locked the code, feel free to experiment.
To enter new member press 'CLEAR' first, to edit existing select from 'listbox'.
