Copy Paste from Form to Historical sheet

I am looking to take cells from the form on "Missed Switch Form" and input them into the historical sheet each time the macro button is hit.

This way it just keeps adding the data to the next open row and just continues each time the form is filled out and button is hit.

Please let me know if there is anything else you need.
