Customize Date field to inlcude additional data

Hello,

I am looking to see if there is a way to modify a date column that is already in a cell and then include a (1) or (2) next to it. I have included my workbook and if you look at it, you will see that column D includes the date that I want to use and then I would like to use the last integer over in column G (ex: DivSemiFinals1) I would like to use the one and have the value turn out as (1). So the first line would have the value in column D to show it as 04/04/84 (1).

If possible, I would like this to be applied on all worksheets within my workbook referring to columns D and G. If anyone could let me know how this can get done I would appreciate it. Thanks in advance for your help.
