Invoke Pivot Table by Coding and Display Fields Data

Hi Xcelperts !!! thanks for this site as all of you are doing a great job........though i am still a learner......and only learning excel now.....many things are new for me... and I need someone to help to create a code to AUTOMATE PIVOT TABLE TO DO THE FOLLOWING AS DATA IS LARGE.....after trying to browse through this site i could not find one that could do automate pivot table the way i want or that I am not able to collate info and do it on my own...and i failed several attempts in doing so....can someone help on this....please see the excel attachment.
