voting poll for internal team excel

Hi All,

I want your help in creating a voting poll in excel userform. There are a list of 12 members for now to whom we have to vote.

Can anyone of you please help in creating a userform which should consist of
Name (the person who is voting) :-_______
Vote for(to whom voting to):______

Cancel or submit. ....button

And the results to no.of votes should be generated in next sheet of same worksheet.

Can you please help.

I was expecting one box to be added
A Comment box (to reason for voting the selected person).

Can you please tell me on how can l add a comment box as well please.
