Delete Duplicate Data from certain cells in specific columns

Greetings.
I have attached a spreadsheet showing the before and after of what I am hoping to accomplish with a macro.

I have data in certain cells throughout my spreadsheet that I need to delete. I want to keep the rows but delete duplicated information in these cells. Since these spreadsheets have hundreds of rows I thought a macro would be best.

Any help will be greatly appreciated. Please let me know if you need further information after seeing the spreadsheet.
