VBA to Export workable PDF of specific sheet.

I have a spreadsheet that I use to create quotes for customers. I now would like to create a button that will open one of the sheets as a PDF. The PDF needs to have certain fields to be unlocked so that they can enter their name and date. I would also like to have check mark boxes on the form as well.

I have been able to get the PDF to open, just need to be able to type in certain fields.

Any help would be greatly appreciated.
