Suggestion on capturing inputs - textbox in userform

Hello,
I have a list of KPIs which I'm populating on a sheet with 3 rows gap between each of them. I would like to insert a text box in that gap so that a user can enter the value for each KPI. I believe I can take advantage of userforms but since my whole code in the module and userforms don't seem to be working when they are in the module, could someone suggest me how to go about it?

Also can textboxes be created using a loop through userforms?

I don't want to have a inputbox for each KPI because there are too many and once wrongly entered it would be difficult to go back and change, unless start the whole exercise again.
