How do I update a the date o a Macro?

Hi guys,


I have a macro that pulls the name and dates (SHEET: "Names) off one and puts it on the other sheet (FORM) - it was all programmed for Jan 1 2015 through Dec 31st 2015. How do I make a new 2016 trainings sheet, and make it tabulate on the "form" SHEET? It works perfectly if you put any thing on it that is 2015, but does not work if it is 2016. Let me know if this is enough information - see attached file.
