Create a form that auto populates another worksheet

I'm having a lot of trouble on this one. I need to create a form that will auto populate a back page. So, when I hit enter on the form, that date on the other worksheet will fill from the form. I've uploaded the workbook. Sheet 1 is what the report should contain. Statistics Spreadsheet is where I would like it to be auto populated to. Any help is great help thanks!
