Merge Excel data into word document(s)

Hi this is my first post! I sure hope someone can help as I have been really struggling with how to get this solved.

Attached is the excel file in question.
The objective is to create a performance review for each person in the "Name of individual Being Evaluated" column. It is very common that the name will come up many time as different "Reviewer" submit reviews for them. I need a macro or add in that can take a file that is formatted in this way and insert relevant values into a template word document. Kind of like how mail merge works except each unique "Name of Individual Being Evaluated" should spawn a separate word document and include the feedback from each reviewer.

Each of the rating column sections should be formatted like this. The Reviewer name on the left and their corresponding rating on the right.
