    I don't know about form boxes. I usually just make a shape, right click on it and select "Assign Macro." See if this works for you.

    I got it sorted with the formula you provided. Just created a "Form" button, clicked "Edit Code" in the developer toolbar...pasted that bad-boy in there...works. *shrug* Appreciate it.
