How to have pivot summary formatted as tabular form

Hi I have a macro which creates a pivot table as shown in attachment.
On clicking the submit button it will generate the pivot summary as shown in "Sheet 1".

But do I get this in tabular form. Basically I need to have pivot to be shown as shown in "Sheet 2".

I know I can get this done by using Macro recorder, but it is not so useful because daily my details will be increasing so I need a VBA code to achieve this ...
please advise.
