.Offset multiple non adjacent cells

I have a file set up to retrieve information from the database and display it in a message box, presently it searches for the value and displays the applicable name, I can't figure out how to get it to show more than on column of data in the message box.

The offset (0,-12) get's the one column, how do I get it to show that and another non adjacent cell together in one line?

If you'r showing the str value in the message box then presumably something like
