Formatting multiple sets of Data within Excel (Very Challenging )

    Hi,

Recently I have come up with this complex question in excel . Can you please provide solution.

I need a macro which can convert data present in input sheet to the format of data present in output sheet.

One more thing is the amount of data is variable in input sheet that is there can be 1000's of data available in input sheet but when i run my macro the data should be formatted as that of output sheet.

PS: Input and Output sheets attached.

    Thanks in advance
